
MEMBERSHIP APPLICATION
Instructions: Complete the online membership application and click "submit". You will be taken to the thank you page. Please read and fill out the required information on the Waiver of Liability page and click submit. You will be sent to Paypal to finish the membership process. The fee schedule is as follows:
MEMBERSHIP FEE WITH NO BACKGROUND CHECK (See below for acceptable documentation): $15.00
MEMBERSHIP FEE WITH BACKGROUND CHECK: $35.00
On the Paypal page, you can pay with a credit card or use your personal Paypal account. Simply fill in the amount to be paid and any other required information.
You may also download and print the application and mail it via "snail" mail to the address listed on the document. You can view and print the application and Waiver of Liability by clicking here. The mailing address is on the forms.

A $15 application fee must accompany this application. If a background check is necessary, an additional fee of $20 is required. Fees must be paid on submission of this form.
